As a member of the Social Work Department you will play an integral role in enhancing the well-being of our residents and help them meet their basic and complex needs.
Responsibilities:
Develop social, psychosocial history, formulate care plan which addresses problems, needs, and concerns.
Participate in Interdisciplinary MeetingsMaintain compliance with MDS assessments, care plan review and revision.
Maintain compliance with federal and state documentation requirements.
Inform, educate, and advocate on behalf of patients and their resident rights.
Coordinate discharge planning, and assess patients for the potential of discharging.
Provide counseling to residentsMaintain compliance with required state assessment schedule, e.
g.
Assessment Pro Level I, IIs, Level of Cares and communicate changes or pertinent information to IDT.
Serve as point of contact with hospice as well communal agencies and care providers for post-discharge environments.
State/Federal Survey experienceParticipate in Quality Assurance and Performance Improvement Initiatives (QAPI) Program.
Communicate with Probate Court in matters related to ConservatorshipsOther Duties as assigned.
Skills:
Must possess a BSW, MSW, or have qualifications to serve as a designee detailed in CT DPH 19-13-D8t Public Health Code.
Must possess working knowledge of Healthcare /SNF/LTC Social Work practicesMust possess strong work ethic, communication skills, and writing skills.
